What the inspector wrote

On 04/21/2026 , Licensing Program Analyst (LPA) Cristina Castellanos conducted an unannounced case management incident inspection to follow up on a self-reported incident that occurred on 04/14/2026 at the above-referenced facility. LPA Castellanos met with Director Netsanet Lessanework and explained the purpose of the visit. LPA observed fifty-seven (57) children in care and nineteen (19) staff members providing supervision. On 04/16/2026, Officer of the Day LPA B. Lovest received a call from Director Lessanework regarding an Unusual Incident Report (UIR) involving child (C1). According to the report, as students were transitioning to go outside in the morning, C1 began to cry and stated that Staff Member 2 had pinched him. The parent withdrew C1 from the center on 04/15/2026; the child’s last day of attendance was 04/14/2026. LPA requested the following documents: the children’s roster and copies of Launch Pad Learning staff statements dated 04/14/2026. Additionally, during today’s inspection, LPA initiated staff interviews. Based on the information obtained, no citations were issued during today’s inspection. The UIR investigation requires further review. An exit interview was conducted, and a copy of this report (LIC 809) and the Notice of Site Visit were provided to Director Lessanework.
