What the inspector wrote

On 05/19/2026 Licensing Program Analyst (LPA) Lisa Clayton arrived unannounced at the Launch Pad Child Care Center to conduct a Case Management – Incident inspection, to follow up on an Unusual Incident that occurred on Thursday 05/07/2026, and was reported to the department on Friday 05/08/2026. LPA Clayton was greeted by Director Netsanet Lessanework and observed 60 children being supervised and cared for appropriately by 12 fingerprint cleared staff. INCIDENT DETAIL: RP stated that S1 told me that she and the other teachers observed bruises on the child’s thigh’s, left shin and left arm. The teachers took photos of the child’s injuries. They called me, so I went into the classroom, and I brought the child into my office. S3 and I observed the bruises on the child thighs, left shin and left arm. The child doesn’t really talk. S3 asked the child “what happened to you” in Spanish and the child did not respond. She stayed with me in the office for between 1 – 1.5 hours and I noticed that she was getting sleepy, so S4 came and took her to class for naptime. I called the teachers (witnesses) into my office and we made the incident report and called the suspected child abuse hotline to make a report. I spoke to Eric Ellis – ref. # 1515-0882-5459-009-2480. Update: the child has not returned to the center since May 7, 2026. During today’s visit, LPA Clayton toured the facility for a Health and Safety inspection, interviewed staff, and obtained a facility roster, personnel report and documents related to the unusual incident. Based on LPA’s observation and record review, Per Title 22, Division 12, Chapter 3, of the California Code of Regulations and/or Health and Safety Codes, staff responded to the incident appropriately and reported the incident according to Child Care regulations, therefore no citations were issued. An exit interview was conducted, the report was reviewed and provided to Director Netsanet Lessanework. LPA Clayton posted a Notice of Site Visit which must remain posted for 30 days.
