Incident investigation·July 22, 2025

On 07/22/2025, Licensing Program Analyst (LPA) Adrian Risher conducted an unannounced case management inspection regarding the unusual incident reported on 07/18/2025. LPA provided the purpose of the visit and met with Madison Sanello, Director. LPA observed 19 children with 4 staff present in the facility. On 07/18/2025, the facility reported an unusual incident involving child 1 falling while playing outside on the yard. Child 1 was injured as a result of the fall. During this inspection, LPA conducted interviews with facility staff and children. LPA received a copy of the roster. Based on the investigation which included interviews with relevant parties and observations by the LPA, it has been determined that further investigation is needed. No deficiencies will be cited today. An exit interview was conducted and a copy of this report along with the Notice of Site Visit were provided to Madison Sanello, Director.
