What the inspector wrote

On 6/5/2025 Licensing Program Analysts (LPAs) Chartrice Johnson and Jillinda Chandler made an announced visit to Beach Moms CDC, Inc. Infant Center located at 1502 20th St, Santa Monica, CA 90404. The facility is housed on the campus of a church who's address is 1518 20th St., Santa Monica, CA 90404. The purpose of today\342\200\231s visit was to conduct a pre-licensing inspection. LPAs met with applicant and owner Celia Fisher. This is an application for a single license for a total capacity of 6 infants. Capacity was determined by fire clearance. The days and hours of operation will be Monday-Friday 7:00 am- 6:00 pm. LPAs confirmed the facility phone number to be (310) 341-6269. This is a one-story building located at the rear of a church. The building contains a classroom and separate sleeping area, an adjacent bathroom, and an enclosed outdoor play area. The building also contains a separate teacher's lounge and staff bathroom. A 2A10BC fire extinguisher is mounted to the wall of the classroom. Applicant was informed that there shall be a Disaster and Mass Casualty Plan readily available for review per Title 22, section 101174 at the center. A dual carbon monoxide and smoke detector was observed in the classroom. First aid kit was available with the required essentials: bandages, ointments, and manuals. Applicant shall ensure first aid kits contain tweezers and thermometer. Age-appropriate furniture and equipment was observed in fair condition. LPAs observed a combination of toys that provided and encouraged: Auditory stimulation, Visual stimulation, Tactile stimulation and Manipulative skills in the infant classroom. Page 1 Cots were observed for napping in fair repair stored in the storage closet of the classroom. LPAs observed 4 cribs in the napping area of the infant classroom. Applicant was informed per Title 22, Chapter 1, Sub Chapter 2, Section 101439 (g), Cribs, mats or cots shall be arranged so as to provide a walkway and work space between the cribs, mats or cots sufficient to permit staff to reach each infant without having to step over or reach over any other infant. Applicant shall ensure that no exits are blocked by crib placement. Drinking water will be provided through filtered water pitchers and bottled water. Children use their personal drinking canisters when drinking water. The facility has central heating and ceiling fans. Per applicant, facility will use a space heater due to heating controls being located in another office on the premises. Windows were in good repair free of chipping paint, dirt, insects, or debris. Adequate lighting was observed. The classrooms were clean in good repair. A separation area within the classroom book area will be used for isolation of ill children. A designated bathroom stall will be used for ill children. Applicant shall ensure there will be a mat and a crib or portable crib readily available for the isolation area. The facility uses a designated phone for the center. Parents and authorized adults will sign children in and out, using the Bright Wheel app. Teachers will communicate with parents and guardians through the application. The required postings were also posted. Applicant shall post license number on all advertisements and signage. Measurements for the indoor activity space for infants was 341.4 square feet, which accommodate the applicant\342\200\231s request. Page 2 FOOD SERVICE: Parents are required to provide all meals and snacks for children in care. Facility will provide water. Applicant was informed that there shall be extra provisions on site for infant children. OUTDOOR ACTIVITY SPACE The center has an outdoor play area for infants which meets Title 22, section 101238.2 (a); There shall be at least 75 square feet per child of outdoor activity space based on the total licensed capacity. The outdoor play yards were gated with a 4 foot or higher gate. Age-appropriate toys and equipment were observed in fair condition. Applicant shall provide a plan for cushioning of the play area. Extra padding is needed to prevent tripping hazards and provide resilient cushioning. LPAs observed shade and seating areas for resting on the infant yard. Measurements for the outdoor activity area was 542.97 square feet for the infants for a total of 7 which will accommodate the requested capacity. LPAs observed a kitchen located within the church that will only be by staff used to rinse bottles for the center. LPAs observed a small refrigerator in the classroom as well as bottle warmers. Foods and toxins or chemicals shall be stored separately, and LPAs did not observe any expired or contaminated foods. This area was clean, free of insects or vermin. Center has devised an Incidental Medical Service (IMS) plan to provide to parents of children with allergies (epi-pen), asthmatic (inhalers) glucose monitoring (diabetics), and children needing G-tube feeding. IMS was discussed with the applicant. Trash cans used for solid waste were observed with tight fitting lids. Page 3 RESTROOMS One changing tables within arm\342\200\231s reach of a sink was observed in the bathroom. There are 3 sinks and 7 toilets available for children use, which will accommodate the requested capacity. Per Title 22, section 101439(i)(1)(2) There shall be a minimum of one hand washing sink to every 15 infants and one potty chair to every five infants being toilet trained. (1) If sufficient potty chairs are not available for the number of infants being toilet trained, a combination of potty chairs and toilets with training seats may be used to meet the ratio specified in (i) above. (2) Infants shall not be permitted to play with potty chairs. The bathroom is made inaccessible to the outside church by a latched door. Per the applicant, the leasing agreement provides the center with exclusive use of the restroom. The restrooms were clean and sanitized with the necessary toiletries, sinks and toilets were operable and in good repair. Faucets delivered cold water and hot water. Applicant turned off the hot water. Based on today\342\200\231s inspection the facility shall be recommended for a maximum capacity of 6 infants determined by the indoor measurements and at the applicants requests, following the recommended corrections, fire clearance, and required documents: Applicant shall obtain a fire clearance Separation for the napping area in the infant room shall be at least 4 feet and sound absorbent Applicant shall ensure that first aid kits contain tweezers and thermometer Applicant will ensure that television is secured to prevent falling Adaptive equipment for toilets and sinks will be placed in the bathroom Applicant and director will attend an infant safe sleeping training Applicant will remove crib to provide access to exit Applicant will ensure that the facility has adequate heating Applicant will ensure refrigerator has a thermometer Applicant will ensure that bathroom has a trash can with a tight fitting lid for waste Applicant shall ensure that rugs do not cause a tripping hazard. Page 4 The following items were discussed: The applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPAs reviewed with facility representative the LIC 311A, Records To be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the applicant shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). This facility provides Incidental Medical Services \342\200\223 IMS. LPAS reviewed storage of medication and equipment/supplies. Applicant was informed to refer to Title 22, Division 12 Chapter 1, Subchapter 02 Infant Care Centers for information pertaining to infant care. Page 5 For IMS information see PIN 22-02-CCP . The applicant was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. LPAS discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep , as an additional resource. LPAS also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
