What the inspector wrote

On 3/12/2026, Licensing Program Analyst (LPA) Devon Carus conducted an announced case management licensee-initiated inspection for the purpose of changing the center\342\200\231s capacity. The applicant is requesting an increase to their pre school license\342\200\231s toddler component from 8 to 21 children ages 18 months \342\200\223 23 months. Classroom 5 and Classroom 8, will be dedicated as toddler classrooms. At this time, Classroom 5 is already licensed for 9 toddlers. The hours of operation are Monday - Friday 7:45AM \342\200\223 4:30PM. The fire clearance was approved on 2/3/2026. LPA met with Shannon Kolb, Director and toured the facility. Upon arrival there were 108 pre school children present. Indoor Classroom measurements are Classroom 8/ proposed new classroom indoor measurements contain a total 472.136 sq ft of which will accommodate Licensee request for 12 additional toddlers. Based on today\342\200\231s measurements, facility indoor space limit is 12 children in the proposed toddler classroom. Outdoor Play measurements are: The toddler area outdoor measurements contain a total of 1263.08 sq ft which will not accommodate the licensee\342\200\231s request for 12 additional toddler children at one time. Based on today\342\200\231s measurements, facility\342\200\231s outdoor space limit is 16 toddler children, which exceeds the facility\342\200\231s current toddler capacity of 9, but does not exceed the proposed toddler capacity of 21. LPA Carus toured the on-limits areas and classrooms located in the facility. The facility has a total of 6 sinks and 4 toilets for the designated classrooms to use. Page 1 Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed before a licensure will be considered. Corrections are due by 4/12/2026 \302\267 Waiver Request for Toddler outdoor activity space, including a detailed schedule of outdoor times for each classroom. An exit interview was conducted with the above items discussed and a copy of appeal rights and this report was provided to the Director, Shannon Kolb. Final license determination will be made upon review by the Licensing Program Manager. Page 5 The following was observed. Parents will sign in and out on physical copies of a sign in and out sheet that is made each day. Providers are to ensure that such records are immediately available upon request for inspection and audit by program staff. The facility provides filtered drinking water that is readily available in each class and on the playground areas. Additionally, parents may provide drinking water for the children as an alternative. There are carbon monoxide detectors located in the classrooms. LPAs observed a First aid kit with the required materials including a handheld digital thermometer, ice pack, gauze, and band aids. Furniture and equipment were observed to be maintained in good condition, free of sharp, Loose or pointed parts. Tables and chairs were observed to meet the needs of the children. All play equipment and materials used by children were observed to be age appropriate. There are cubies, in all classrooms for storage of children\342\200\231s clothing and personal belongings The classrooms have portable air conditioners and central heating. The classrooms were clean, and in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephones in the office area. Page 2 Medication is stored in a locked cabinet in the staff office and is inaccessible to children. LPAs observed disinfectants, cleaning solutions, and other items that are dangerous to children on high shelves in the classrooms and are inaccessible to children in care. The required postings are posted in a common area for parents and visitor\342\200\231s review. The staff office will be used as an ill/isolation area. LPA observed fire extinguishers with valid inspection dates. The program has the parents of enrolled children provide lunches, and will center provides snacks twice per day. Additionally, there are back up snacks and food items on the premises. The facility has an Incidental Medical Service plan in the event that services are needed. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the outdoor activity area. LPA observed outdoor shaded areas. LPA did not observe any bodies of water on the premises. There are no firearms on the premises. Page 3 The following items were discussed: Director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. LPA reviewed with facility representative the LIC 311A, Records to Be Maintained at The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP . Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP . When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
