What the inspector wrote

On 02/11/2026,  Licensing Program Analyst, LPA, Tyra Chavies conducted an unannounced case management- incident visit to follow-up on a self- reported unusual Incident (LIC 624) reported (UIR) to Community Care Licensing Division (CCLD) on 11/05/2025. LPA Chavies toured the facility and met with principal, Lisette Sepulveda . There were 36 children being supervised and cared for by 10 staff members. During today's visit, LPA Chavies observed children in care and conducted in person interviews with S#2. Based on the information obtained the UIR requires further investigation. An exit interview was conducted, report was reviewed and provided to principal. Notice of site visit was provided and must be posted for 30 days.
