What the inspector wrote

On Tuesday, April 28, 2026, during a complaint visit, Licensing Program Analyst (LPA) Mayra Rivera reviewed children files and observed child #7 and child #8 not have a file on premises. LPA Rivera observed 4 infants and 4 preschool children and co-licensee Edith. LPA observed the facility to be operating within ratio. LPA Rivera also observed child #1, child #2, child #5, child #7 and child #8 not have immunization record. Licensee Brigitte stated could not find the files for child #7 and child #8 and thought she had the immunization records for all children. The deficiencies listed on the following page were observed by LPA Rivera and are being cited in accordance with California Code of Regulations Title 22. Two Type B deficiencies are being issued today for regulations: 102421 Child's Records and 102418 Immunizations . Please see attached LIC 809-D for citation. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with Licensee Brigitte Grijalva.
