What the inspector wrote

Based on LPA observations and interviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. A Type B citation issued. A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the licensee Tomasa Serrano.

This is a Type B violation that could pose a potential risk to children in care.
