On 7/17/25, Licensing Program Analyst (LPA) Crystal Ali met with Brenda Colmenero Licensee, conduct an unannounced case management inspection. The purpose of the case management (CM) was to follow up on unusual incident report (UIR) received 6/12/25. Incident occurred on 6/12/25 at approximately 9:43am, child#1 was injured during water play. Child #1 went down the blow-up slide incorrectly and hurt her back and was unconscious for approximately 7 seconds. 911 called and it was determined child #1 had the wind knocked out. Upon arrival, LPA observed 13 daycare children, 1 infant, licensee, assistants (2), providing care and supervision. During this inspection LPA interviewed Licensee, assistant #1, and Child #1. LPA called parent of child#1 and left voicemail message. LPA obtained a copy of the LIC 9040 and did a tour of facility. Due to the need to gather additional information, the case management will require further investigation. Exit interview conducted with Brenda Colmenero and Notice of Site Visit. Notice of site visit must remain posted for 30 days.
