On 8/5/25, Licensing Program Analyst (LPA) Crystal Ali met with Brenda Colmenero Licensee, conduct an unannounced case management inspection. The purpose of the case management (CM) was to follow up on unusual incident report (UIR) received 6/12/25. Incident occurred on 6/12/25 at approximately 9:43am, child#1 was injured during water play. Child #1 went down the blow-up slide incorrectly and hurt her back and was unconscious for approximately 7 seconds. 911 called and it was determined child #1 had the wind knocked out of her and there was no need to transport child to hospital. Paramedics recommend parents to follow up with child primary doctor. Upon arrival, LPA observed 8 daycare children, licensee, and one assistant providing care. During this inspection LPA concluded the CM for UIR incident dated 6/12/25. All gathered information and interviews concluded that licensee responded correctly in accordance with Title 22 and Health and Safety Codes. No further investigation is need. Exit interview conducted with Brenda Colmenero and Notice of Site Visit. Notice of site visit must remain posted for 30 days.
