What the inspector wrote

On 3/14/22, Licensing Program Analyst (LPA) Zirbes conducted an announced Pre-Licensing inspection. LPA met with applicant Ani Ekmekchyan. The purpose of the inspection was to ensure the facility meets basic licensing requirements set forth by Title 22, Health and Safety and statutory requirement. The applicant is requesting to provide care for a Small Family Child Care Home (FCCH) with a capacity of 8 children. Currently, there are two adults (applicant, A1) and two children ages three and 6 months living in the home. LPA toured the home inside and out to ensure it meets FCCH requirements. Licensee is requesting the days and hours of operation will be Monday through Sunday, 24 hours a day. The home is described as follows: This is a two story home consisting of four bedrooms, three bathrooms, office, laundry room, kitchen, dinning room, living room, playroom, lounge area, attached garage and front and back yard. The off-limits areas are: the entire second floor of the home (all bedrooms, master bathroom, bathroom 2, office, laundry room), attached garage. The off-limits areas have been made inaccessible via a child safety gate at the bottom of the stairs and the door leading to the garage has a dead bolt lock and a chime which rings when the door is opened. On limit areas: Playroom: (child care room): This area is located on the first level of the home, to the left of the front door. The area had a couch, child size table and chair, television, and a shelving unit containing toys (dolls, balls, blocks, books). Lounge area: This area is located on the first floor of the home to the right of the front door. At the time of this inspection there was an eight person table/chairs and a high chair in the room. Bathroom #1 : The bathroom is located on the first floor of the home. The bathroom had a sink, toilet, and shower. The bathroom was toured and inspected. Toilet and faucet were clean, safe, and operable, shower area was free of hazard. Neither medications nor chemicals were stored in this area at the time of the inspection. Continued on page two Continued from page one Living room: This area is located on the first floor of the home. The room has two sofas, table, book shelves, and a television. Dining Room: This area is located on the first floor of the home. The area had a child size table and four chairs. Per applicant, the dining room will be utilized as the napping area. LPA observed two exersaucers and an inclined sleeper in this area. Applicant stated she is aware that items can not be utilized by the childcare children or her children during child care hours. There is a bar area in this room. LPA expressed that alcohol must be inaccessible to the child care children. Applicant stated she understood. Kitchen: This area is located on the first floor of the home. All sharp utensils and cutlery, cleaning supplies, drawers, and cabinets with plastic bags, and sharp things or small things children can swallow; were made inaccessible with child safety locks. The chemicals were stored in the cabinet under the kitchen sink which was equipped with a child safety lock. Knives were stored in an upper cabinet of the kitchen. The refrigerator, dishwasher, stove, microwave, etc., were clean. The kitchen was clean, orderly, and free of hazardous items. Outside: The yard was inspected. There are no bodies of water on the premises. The yard has a shaded patio, a cement area to ride bikes/scooters, and a grass area. There was climbing equipment placed on the grass. The climbing equipment had one slide and two swings. At the time of this inspection, the climbing equipment was secured to the ground. LPA observed an air conditioner unit on the side of the home which was accessible to the children who will be in care. LPA requested the applicant purchase a cover to make the air conditioning unit inaccessible to the children in care. LPA observed an outdoor refrigerator with alcoholic beverages. LPA and applicant discussed that alcohol must be inaccessible to the child care children. Per applicant these items will be removed or the refrigerator will be locked. The home has central heating and air conditioning. All windows and screens are free of cracks, bugs, and debris. At the time of this inspection, the home did not have window blinds with cords. Napping equipment (cots): At this time the applicant does not have napping equipment. Applicant has agreed to obtain napping equipment to meet the needs of the children in care. Per applicant, the children will nap in the living room/dining room located on the first floor of the home. Report continued on page three Report continued from page two Electrical outlets: All unused electrical outlets are plugged in and made inaccessible to children. The First Aid kit : Located in the bathroom on the first floor of the home. The first aid kit was equipped with the required supplies at the time if this inspection. Weapons or Firearms : Per the applicant, there are weapons or firearms. Weapons were stored within a gun safe on the second floor of the home within the master bedroom in the off-limits area. During this inspection the applicant opened the gun safe, and LPA observed the firearms and the ammunition were located in the same compartment of the gun safe. LPA explained the ammunition and firearms had to be in separate locked locations. LPA provided applicant with a copy of t he Operation of a Family Child Care Hom e 102417 (g)(4) . Applicant stated she will move the ammunition to a separate locked area. Cleaning compounds : Located with the cabinet under the kitchen sink. This cabinet door was equipped with child safe lock. Cleaning supplies were also located in the locked garage. A smoke detector and a carbon monoxide detector : Located on both floors of the home. During this inspection smoke detector and carbon monoxide were tested and found to be operable. Fire extinguisher: There is a fully charged 1A10BC fire extinguisher. LPA explained the fire extinguisher must be a 2A10BC. Applicant stated she will purchase a fire extinguisher which meets the regulations. Pet: No pets were on the property at the it of this inspection. Off-limit areas Entire second floor of the home. The area is inaccessible via the use of a child safety gate at the bottom of the stairs. LPA discussed the child safety gate must always be closed to prevent children from accessing the stairs and the off limits area. Applicant stated she understood. Master Bedroom/Master Bathroom: Located on the second floor of the home. The bedroom is occupied by Applicant and Adult 1. LPA observe a bed, dresser, closets and personal furnishings. Bedroom 2: Located on the second floor of the home. The bedroom is occupied by child 1. LPA observed a bed, dresser, closet and personal furnishings. Bedroom 3: Located on the second floor of the home. The bedroom is occupied by child 2. LPA observed a crib, dresser, closet and personal furnishings. Bedroom 4: Located on the second floor of the home. The bedroom is unoccupied and is used as extra storage space for the family. Report continued on page four Report continued from page three Bathroom 2: Located on the second floor of the home. The bathroom has a toilet, sink, shower. Office: The area is located on the second level of the home. The area was furnished with a desk, and storage cabinets. Documentation: Applicant has current CPR, First Aid Training with the expiration dates of 1/25/2023. Prevented Health and Safety Training was completed on 09/17/2016. Preventative Lead training was completed on 2/5/2022. Mandated Reporting Training dated 2/9/2022. Per applicant, transportation will not be provided at this time. Applicant has fingerprint clearance, TB exam, and proof MMR, and influenza. Per applicant meals and snacks will be provided for children. The following was discussed with the licensee: LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children\342\200\231s forms/records, facility forms/records, and information to be posted. Applicant was reminded that 100% supervision is required at all times to children in care. Applicant was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov . Applicant was made aware that it is he/her responsibility to know the regulations as well as anyone who assists in providing care. Applicant must have the facility\342\200\231s phone number at all times; if the phone number is changed, licensing must be notified The regulation prohibits the smoking of tobacco in a private residence that is licensed as a family childcare home and in those areas of the family day care home where children are present (24/7 ban). State law prohibits baby walkers, bouncy seats, exersaucers, and any other items that fall into that category. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. On January 1, 2018, or before March 30, 2018, a person who is a licensed childcare provider, administrator, or employee of a licensed child day care facility shall complete the online mandated reporter training and shall complete renewal mandated reporter training every two years. @www.mandatereporterca.com. Report continued on page five Continued from page four *Incidental medical Services (IMS) policy was discussed. Applicant will wait until facility opens to determine IMS needs. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment Lead Poisoning: For more information, go to the California Childhood Lead Poisoning Prevention Branch\342\200\231s website at www.cdph.ca.gov/programs/clppb,or call them at (510) 620-5600. Licensee advised visit www.shotsforschool.org for Immunization information . Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. As a result of this inspection, the home requires the following to be completed: Firearms and ammunition must be locked and stored separately from each other, paint on the side of the home must be relocated, alcoholic beverages must be inaccessible, remove inclined sleeper and exersaucers from on limits areas, purchase a 2A10BC fire extinguisher. Applicant stated all corrections will be completed by March 17, 2022. Exit interview conducted and report was reviewed with the applicant Ani Ekmekchyan.
