On Wednesday, August 20, 2025, Licensing Program Analyst (LPA) Evelyn Garcia conducted an unannounced annual inspection and met with licensee Seda Yeghiazaryan who guided LPA Garcia on a tour of the facility. During the visit, there were 12 children (2 infants, 2 toddlers, 6 preschool children, 2 school children) present and Licensee was alone with the children placing her out of ratio. Licensee was unable to contact her employee who stepped out for a doctor\342\200\231s appointment. LPA informed licensee that parent\342\200\231s will have to be notified to pick up their child so she can be in Ratio. The childcare hours of operation are Monday through Friday 7:00 am to 12:00 am. Licensee lives in the home with her 2 minor children. This is a one-story house with 3 bedrooms, and 2 bathrooms, living room, family room, and kitchen. Main care is provided in the family room area (back of home). The children eat in the family room area. are no bodies of water on the premises. Areas off limits are the living room, bedrooms #2-3, bathroom #2, kitchen, and front yard. Areas accessible to the children are the family room (back of home), bathroom inside bedroom #1, located next to the family room, back yard for outdoor play. The children and parent\342\200\231s use the backdoor located in the backyard for arrival and departure. LPA Garcia inspected the facility for safety, comfort, cleanliness, ventilation and working phone (cell phone). For ventilation, LPA Garcia observed central AC and the vents located on the ceiling. LPA observed the furniture, children\342\200\231s materials, to be in good condition and age appropriate. The dual smoke and carbon monoxide detectors were observed in the ceiling of the main day care room. Licensee tested the dual smoke and carbon monoxide detector. LPA heard the sounds and is operable. LPA observed a fully charge 1A10BC fire extinguisher during the inspection located in the family room near entrance door. LPA informed licensee to purchase the required 2A10BC or 3A40BC and email LPA proof of purchase. LPA Garcia observed the First aid kit is stored in the bathroom. LPA observed first aid kit complete with gauze, antiseptics, and bandages. License stated medications are stored in the licensee\342\200\231s bedroom and off limits to daycare children. Cleaning items are inaccessible to children stored in a kitchen cabinet above the microwave and kept separate from food. Kitchen: The kitchen was observed to be clean and orderly. The following are inaccessible: Sharp items are stored in an kitchen cabinet, unreachable to children in care. LPA observed the refrigerator and freezer to be clean. LPA discuss with applicant food shall be properly stored or refrigerated in container that are labeled with child\342\200\231s name when supplied by parent. The kitchen will be off limits to children in care. Applicant is interested in participating in a food program. LPA Garcia inspected the play outdoor (back yard) area that is utilized by children for safety, comfort, and cleanliness LPA observed it to be free of garden tools, poisonous plants, thorn trees cactus, or lawn mower inaccessible to children. LPA observed the outdoor air conditioning unit and electrical box to be completely blocked off by a gate and secured with a latch. The back yard is enclosed with a vinyl fence. LPA observed the play equipment to be age appropriate and in a safe condition, free of sharp, no lose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards (artificial turf). For outdoor water drinking, children bring out their cups and refill as needed. LPA Garcia observed cleaning compounds items to be stored under a cabinet in sink in the kitchen and in a cabinet above the microwave. The children have no access to the kitchen as it is closed and locked during daycare hours. Knives and sharp objects were observed to be stored in a high kitchen cabinet that is out of reach to the children and inaccessible to the children. For water drinking, licensee stated he provides filtered water, and each child has their own labeled re-fillable cup. The licensee stated the facility provides breakfast, lunch and p.m. snacks and does not participate in a food program. Licensee stated she currently does not have any children with severe food allergies nor on medication. LPA Garcia entered the bathroom in the main day care area (Family room area) and observed the toilet, hand washing sink, hand soap and LPA observed the cabinet under the sink and did not observe hazardous cleaning materials to be stored in the under-sink cabinet. LPA observed the restroom to be in good, operable condition. LPA Garcia asked the licensee if there were any pets, poisons, firearms, weapons, or bodies of water. The licensee stated she no pets, no poisons, no firearms, and no weapons in the home. LPA did not observe pets, poisons, firearms, nor weapons. The license was informed that if any poisons (ex; Drano, rat poison or items that fall into that category), firearms and weapons are purchased, it is required to be locked with a key or combination lock and firearm and ammunition must be stored separately.For ill isolation, licensee stated she utilizes a corner in the main day care area until the child is picked up by parents. Licensee stated she recently conducted an emergency drill but was not aware she had to document it. LPA observed Pediatric First Aid/ CPR certification dated 1/27/25. Licensee did not have proof of immunization against MMR, TB or flu. Licensee has completed the Child Abuse Mandated Reporter (AB 1207) training dated on 11/16/24. Licensee was advised that the mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com LPA did not observe the required postings/ documents: LIC 610A Emergency Disaster Plan, Pub 394 Notification of Parents Rights, LIC 999 Facility sketch. LPA did not observe a disaster drill log, facility roster, Sleep log for infants under 2 years of age. Licensee will purchase required fire extinguisher stated in report above. Licensee will email proof of the documents listed above to LPA Garcia. The following was also discussed with the licensee: 1. In the absence of the licensee a qualified adult must be present, supervising the children; a qualified adult is an individual who has a valid and current Pediatric first aid/ CPR-adult-child- infant certification (EMSA approved), a valid criminal record clearance associated to the facility license, immunization's (MMR, TDAP, TB and Influenza or Influenza declination), AB 1207 Child Abuse Mandated Reporter Certificate. 2. A current roster of children enrolled must be available and maintained for a period of 3 years, even after children are no longer attending the facility. 3. Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the license shall be terminated. 4. The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary and smoke and carbon monoxide detectors should be checked, and batteries replaced as needed. 5. Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home. 6. Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing (refer to LIC 624B). Mandated reporter requirements were reviewed and explained. 7. Fire and safety drills must be performed every six (6) months and documented for review by the Department. 8. Smoking is prohibited in the family childcare home. 9. Children and staff records must be maintained and updated as needed and be available for review by the Department. 10. Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee and all adults working with children have proof of immunizations. 11. Inspection Authority: All adults living and working in the home shall be made of aware of the Department\342\200\231s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation. 12. The facility license number must be on all advertisements, publications, or announcements with the intent to attract clients. 13. Isolation for Ill children: When a child is ill, he/she shall be separated from other children (reference 102417(e) Operation of a Family Child Care Home). 14. Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information. 15. Dog(s) and/or pets are recommended to be isolated from children in care. 16. No baby bouncers, no infant walkers, no Johnny jumpers, no saucer chairs, and any other item that falls into this category is not permitted in the facility. Criminal Record Clearance - Family Child Care Homes Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Safe Sleep - Child Care Centers and Family Child Care Homes LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. There was 1 Type A violation issued during this visit for licensee being alone with no assistant with 12 children present under her care and supervision. Upon receipt of a Type A deficiency, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224. If these requirements are not met civil penalties per violation will be assessed.
