Inspector’s narrative

What the inspector wrote

According to licensee no injury report was required because there was no injury. According to licensee child was going to fall and staff attempted to keep child from getting hurt. There were no disclosures of any child being mishandled in a rough manner and not reporting a child's injury. According to facility children were provided with candy and other treats as a positive reward. Licensee is aware when sweets or any kind are administered to children consent and original packaging is required to avoid any confusion. Based on the information obtained, there is not enough evidence regarding the allegations. Therefore, the allegations are deemed to be unsubstantiated. This agency has investigated the complaint. At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time. Exit interview was conducted with licensee Sheena Ramirez. The licensee was provided a copy of the appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative.
