What the inspector wrote

On Tuesday, August 19, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management -Deficiencies inspection to follow up in regards plan of correction for the deficiencies given on June 6, 2025, Annual inspection. On June 6, 2025, LPA Rivera issued four deficiencies Title 22 and Health and Safety. 102417(g)(9)(A)1 - licensee did not comply with the section cited above in not providing a copy of the emergency drills which poses a potential health, safety or personal rights risk to persons in care. 102421(a) - record review the licensee did not comply with the section cited above in not having child 3, child 4, and child 5 files which poses a potential health, safety or personal rights risk to persons in care. 102425(j)(2)(D)(c)- record review, the licensee did not comply with the section cited above in not completing the 15 min sleep logs for child 1 which poses a potential health, safety or personal rights risk to persons in care Health and Safety: 1596.866(b)(1)- record review and interview the licensee did not comply with the section cited above in not renewing the child abuse mandated reporting certificate. Certification expired on 3/9/2025] which poses a potential health, safety or personal rights risk to persons in care During today's visit, LPA observed safe sleep logs for child #1 from June 7, 2025 to July 23, 2025. Child #1 last day July 23, 2025. LPA observed licensee Child Abuse Mandated Certificate dated 6/22/25. Emergency drill conducted on 5/15/25. LPA Rivera cleared the deficiencies. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with licensee Alma Gonzalez. The licensee was provided a copy of the appeal rights (LIC 9058) and her signature on this form acknowledges receipt of these forms.
