What the inspector wrote

On Wednesday, October 29, 2025, Licensing Program Analyst (LPA) Hanna Cha met with licensee Narine Grigoryan. The purpose of the visit was to conduct an unannounced Annual/Random inspection of the facility. Upon arrival, LPA disclosed the purpose of the inspection to the licensee and toured the facility. At time of arrival, licensee and assistant was providing care and supervision to one infant and nine preschool-aged children. The current days and hours of operation are Mondays through Fridays; 6:00am-6:00pm. Physical Plant/Indoor/Childcare Area : This is a one-story home with three bedrooms, two restrooms, and a back yard. This home consists of a living room, dining room, and kitchen. Main care is provided in room #1 and room #2 referred to as the day care classroom areas. The children utilize the restroom located in room #1. The off-limits areas are bedrooms #3, one restroom, part of the living room, and the kitchen. Childcare Bathroom: The childcare bathroom (located in room #1 to the left) was observed to be in safe and operating condition. Toilet and faucet are in good operable condition. LPA reminded licensee to keep shampoos, mouthwash, razors, toothpaste, and other hazardous items inaccessible to children. Kitchen: The kitchen (located to the left from the front door) was observed for safety, sanitation, and good repair. LPA observed knives and sharp items stored in cabinet made inaccessible with child safety latch. LPA reminded Licensee, if food is brought from the child\342\200\231s home, the container shall be labeled with the child\342\200\231s name and properly stored or refrigerated. Licensee participates in the food program and provides all meals during operational hours. Outdoor/Children\342\200\231s Play Area/Bodies of Water: The children utilize the backyard for outdoor play. LPA inspected for safety, cleanliness, good repair, age-appropriate toys/equipment, and shady areas. The backyard is enclosed. Per LPA observation, no in-ground or above-ground pool on premises. LPA observed one wading pool filled with water in off-limits backyard area (made inaccessible with child safety gate). Per licensee, wading pool of water was used for gardening. Per licensee, children do not have access to wading pool. During the inspection, licensee emptied wading pool of water. LPA informed licensee that all bodies of water, including wading pools must be emptied and hung dry after each use. A Technical Violation issued. Other: First aid kit on premises. Fire extinguisher (2A10BC) observed fully green. Per Licensee, there is no smoking in the home. Last disaster drill conducted on 10/10/2025. Electrical outlets made inaccessible. There is a working telephone (cell phone). The carbon monoxide and smoke alarm in operable condition. The cleaning products stored in high upper shelf, made inaccessible to children. Per licensee, there are no firearms/weapons on premises. File Review: LPA reviewed one child file which included required documents. LPA reviewed licensee\342\200\231s file and assistant\342\200\231s file which included all required documentation. The facility roster maintained current. Licensee Pediatric CPR/FA expires on 06/2027. Licensee Mandated Reporter training expires on 11/2027. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Any duly authorized officer, employee, or agent of the Department shall, upon presentation of proper identification, shall inspect the facility. The Licensee shall permit the Department to inspect the family childcare home, and to privately interview children or staff, to determine compliance with or to prevent violations of family child care laws or regulations. Also, to enter and inspect any place providing personal care, supervision and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation. Licensee advised of the requirement to report Unusual Incidents. Licensee informed to utilize the Unusual Incident Report/Injury Report (LIC624B) when submitting a report to the department at: www.unusualincidentreport@dss.ca.gov . A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven (7) days following the occurrence of any events specified above. LPA discussed safe sleep regulations and the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, licensee Narine Grigoryan confirmed that there are no Registered Sex Offenders residing in the facility. During this inspection, one Technical Violation issued. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with licensee Narine Grigoryan along with Appeal Rights.
