On 02/27/2026, Licensing Program Analyst (LPA) Justeene Tamayo and Carly Hansford (Supervisor I), conducted a annual/random required inspection with licensee Alyssa Garcia ,who guided LPA on a tour of the facility. Upon arrival, LPA observed 1 infant, 1 school age, and 6 preschool age children, along with assistant #1. This is a one-story house with 2 bedrooms, 2 bathrooms , living room, kitchen, laundry room area and garage. Family members residing in the home include 1 adult (licensee) and zero minor child. All adults have been fingerprint cleared and associated. LPA did not observe a pool or bodies of water on the premises. The facility operates Monday through Friday from 6AM-8PM and with a license capacity of 14 children. A Fire Clearance has been granted effective 03/19/2025. Main care is provided in the living room, and bedroom #1. Children eat in the living room(classroom 1) as identified on the facility sketch. Per licensee, she provides meals for the day care children. A food program is not being utilized at this time. The day care children utilize the bathroom located in bedroom #1. Children use rear yard for outdoor play. The front yard, bedroom #2(key locked), bathroom #2(key locked), and garage are off limits to the day care children. The garage is located through bedroom #2(key locked). LPA observed day care area to be clean and orderly, fireplace screened and barricaded in living room, central air and heating, age appropriate toys and play equipment, working smoke detector and carbon monoxide detector. LPA did not observe any hazardous items in the child care bathroom. LPA reminded licensee the children's bathroom must be free of shampoos, mouthwash, medication, perfumes, razor, air freshener, nail polish and polish remover. LPA observed a clean, safe and operable toilet and faucet. LPA observed a fully charged 2A10BC fire extinguisher during the inspection located in main care area by the front door. Per licensee, alcohol is inaccessible with safety gate in kitchen area. First aid kit is stored in the main care area. LPA did not observe any medications in the home. Per licensee, she does not have any medications in the home. Licensee made poisons and cleaning items inaccessible to children stored under the kitchen sink. The kitchen is fully barricaded by a safety gate. Kitchen: The following are inaccessible: Sharp items are stored in the upper kitchen cabinet(safety gate barricading kitchen. LPA observed the refrigerator and freezer to be clean. LPA discuss with licensee food shall be properly stored or refrigerated in container that are labeled with child\342\200\231s name when supplied by parent. LPA toured the backyard and observe it to be free of garden tools, poisonous plants, thorn trees cactus, or lawn mower inaccessible to children. The backyard is completely fenced with wooden fencing. LPA observed outdoor play equipment in operable condition. The outdoor air-conditioner is completely screened with a mesh covering. LPA observed a rock climbing hill near the back yard that is barricaded by a long safety gate and will remain off limits to the day care children. There is also a white storage shed where children do not have access to. There is one cat on the premises in off-limit bedroom. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Documents Provided and or Discussed: The following were provided to comply with Title 22 requirements: Fire Drill Log, roster, Safe Sleep PIN 20-24-CCP, LIC9227 Individual Sleeping Plan, Safe Sleep log, required postings, and list of CCLD forms required for child file each child in care (LIC 311D). Licensee stated currently does not have childcare insurance. Child Care Advocates information: www.childcareadvocatesprogram@cdss.ca.gov Licensee inform smoking is prohibited, applicant stated no one smokes in the home, LPA discussed Health Section 1596.846(b) and (c)\342\200\223102417 (g)(10) and provided licensee with a visual copy of prohibited items. LPA did not observe prohibited items during the inspection. Per licensee no firearms are present in the home. Licensee was informed her cell phone shall be available and charged at all times during daycare hours. Incidental Medical Services (IMS) policy was discussed, informing licensee when any IMS is provided, a plan for providing IMS must be submitted to the Department prior to providing care to a child that need IMS. The plan shall state the type of IMS the facility will be offering, stating the person providing care has been trained to provide the named IMS. The plan will also provide the steps that will be taken when IMS is provided to a child. Licensee was informed all suspected Child Abuse should be reported to the Child Abuse Hot-line at 1-800-540-4000. The above incident should be reported on the form LIC624B per the regulation. The form should be email to unusualincidentreport@dss.ca.gov Advisory/Other: Licensee First Aid/CPR are current and expires 03/18/2027. Preventative Health & Safety training is completed. Mandated Reporter Training expires 04/03/2026. Electrical outlets are inaccessible to children. Last fire disaster drill is maintained current. Last Fire/Disaster drill was completed on 09/03/2025. From staff file review, assistant #1 did not have proof of measles on site, however licensee advised assistant #1 has it and will send the measles vaccination within an hour from CVS. (Technical Violation given). From children file review, child #3 was missing LIC627 and child #7 needs LIC995 filled out. (Two Technical violations given) Children nap on mats in the main care areas. Licensee aware no infant shall be swaddled, and car seat shall not be used for sleeping. Licensee is aware to supervise infants while they are sleeping by physically checking every 15 minutes and documenting the infant\342\200\231s status. Licensee should refer to regulation 102425(J) for documentation requirement. If the infant\342\200\231s Individual Infant Sleeping Plan [LIC 9227 (3/20)] does not have Section C completed, the provider shall return the infant to their back for sleeping. LPA discussed the safe sleep regulations with applicant, including Safe Sleep PIN 20-24-CCP and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. No deficiencies have been cited, however 3 technical violations have been given during today's visit. An exit interview was conducted, and a copy of this report was read and provided to the licensee, along with a copy of her appeal rights and Notice of Site Visit.
