What the inspector wrote

On 04/17/2026, Licensing Program Analysts(LPAs) Justeene Tamayo and Sara Rajapakse Montolla met with licensee Marta Flores de Rafael to conduct a follow up unannounced case management incident inspection. The purpose of the case management was to follow up on unusual incident report (UIR) received at the Palmdale Regional Office on 04/01/26. Upon arrival, LPA observed 2 preschool age children and 1 school age with licensee and assistant #1. Description of incident: On 03/30/26, Child #1 and Child #2 were playing with a ball and Child #1 ran to get the ball and fell and broke two teeth. During the inspection, LPAs interviewed Children #1–2 and Assistant #1. Interviews revealed that at approximately 5:30 PM on 03/30/2026, Children #1 and #2 were playing outside with a soccer ball. Child #2 threw the ball, and as Child #1 went to retrieve it, they tripped and fell, causing their face to hit the concrete. Shortly afterward, Parent #1 arrived to pick up Children #1–2, and the licensee informed Parent #1 of the incident. On 04/01/2026, Parent #1 observed swelling on Child #1’s face and took the child to urgent care. Medical staff confirmed that Child #1 had broken two teeth. No treatment was required at urgent care; however, a follow-up dental appointment was scheduled for 04/10/2026. Child #1 has since returned to normal activities. This incident is deemed accidental, therefore no deficiencies will be cited at this time. The licensee is informed to report any unusual incidents within a timely manner. An exit interview was conducted, and a copy of this report was read and provided to licensee, along with a copy of her appeal rights and Notice of Site Visit.
