Inspector’s narrative

What the inspector wrote

On April 2, 2025, Licensing Program Analyst (LPA) Calloway made an unannounced inspection to the above facility for the purpose of conducting an Annual Random inspection. LPA met with facility representative who granted access. LPA and representative toured the facility inside and outside. LPA observed thirty-seven (37) children in active play and four staff. The facility operates from 2:00 PM to 7:00 PM. Monday-Friday. All employees have a Criminal Record Clearance and are associated with the facility. Physical Plant: There is one clean and safe classroom with enough seating for all children. The telephone service, heating, ventilation, and lighting are adequate. There are cubbies for children's belongings that are labeled with the children’s names inside the classroom. The furniture, equipment, and materials were age appropriate. There is a large inside gym for active work out and play, LPA observed a built-in trampoline area that was supervised and work out bags. There is cushioned mats underneath. Upstairs was a large, padded work out floor for the advanced workout out and older children ages 10-17 years old. Drinking water is available inside the classrooms in the form of water dispenser and their own labeled, water bottles or water fountain. There is no napping. First Aid Kits were observed inside the care room. There is an Emergency Survival Kit kept inside the staff lounge it was observed sealed. Per representative, the fire extinguisher, smoke detector and carbon monoxide detector are operable. Trash cans have secure lids. Bathrooms: There are two separate bathrooms for boys and girls. The girl’s restroom has four stalls, and two hand washing sinks that are functioning properly and age appropriate. The boy’s restroom has two stalls, two urinals, and two hand washing sinks. LPA did not observe any hazards. There was soap, toilet paper, and paper towels readily available. The staff restrooms are separate, upstairs. Kitchen: There is no kitchen only a refrigerator and microwave. Per representative, food and snacks are not provided, and children bring their own food and snacks. The chemicals and cleaning supplies are kept inside a cabinet in the main gym locked. Outside: There is no outside play. The building is fenced and secure. Per representative, there are no bodies of water are on the premises. LPA did not observe any bodies of water. All the storage sheds were locked. Other: The children and staff files were reviewed. There are missing, required forms for the staff files. The required postings LIC 610, PUB 393, PUB 269, LIC 613A was missing from the wall. Per representative, they were not aware they needed to be posted. Current Pediatric CPR and First Aid (expired 5/3/2025) but it is the wrong certification. None of the staff providing care had an acceptable CPR training. Transportation is provided. Per representative, the Disaster Drills: Fire Drill was conducted: 3/3/25 Earthquake drill was conducted: 3/3/25, but there was no documentation for the drills. Per representative, they were not told they needed to document the drills. The Child/Parent Roster was not complete with all required information the physician’s names and phone numbers were missing. Per representative, they were not aware the information needed to be on the roster. The mandated reporter training was reviewed. Per representative, the last fire inspection was conducted on 3/8/2024. The Staff/Child ratios were observed to be compliant; the sign in/out sheets were, paper and legible. Per representative, the children are inspected for illnesses as they arrive. There is a separate area for isolation and care of ill children in the staff lounge room until the parent can pick up the child. Per representative, the medications are stored in the front desk that is kept locked. A review of medication policy indicated that prescription medication is administered only with parent's written permission. The management staff administers medication and documents the dosage, date, and time onto a log. Medication is stays locked inside the front office desk. Medication is properly labeled and stored in its original container. Facility representative was reminded to access the CCLD Licensing website at www.ccld.ca.gov . to obtain information about the most recent regulatory changes and the Quarterly Updates. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5 years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Or LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information. Facility representative was informed of the MyChildCarePlan.org website, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. There are deficiencies cited during this inspection and Technical Violations. See LIC 809D and TV pages attached to this report. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, this report, and Appeal Rights were provided to Nicole Vizcarra, Facility Representative. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
