On 01/10/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conclude the gastrointestinal outbreak from the first original UIR received at Palmdale Regional Office on 10/04/24 . LPA disclosed the purpose of the inspection. Upon arrival, there are 7 toddlers and zero infants, along with 7 teachers. The facility has been cleared of the gastrointestinal outbreak by the Department of Public Health on 11/12/24, and there are no more known cases. The director is encouraged to continue to report unusual incidents to the Palmdale Regional Office. No deficiencies have been cited at this time. An exit interview was conducted, and a copy of this report was read and provided to Director on this date, along with a copy of her appeal rights and Notice of Site Visit.
