What the inspector wrote

On 02/11/2026, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conduct a UIR follow up regarding UIR received 01/30/2026 and occurred on 01/30/2026 . LPA disclosed the purpose of the inspection. Upon arrival, there are 15 infants/toddlers in care, along with 8 teachers and Director. Summary of incident: child#1 fell during indoor play around 11:20 received a bump and first aid was applied. Parent #1 was notified, and parent #1 arrived to pick up child #1 at 12:07. During the inspection, LPA Tamayo interviewed Teachers #1 and #2, Family Advocate #1, and Parent #1. LPA was unable to interview the children due to their age. Based on the interviews, Child #1 and Child #2 were playing inside the classroom when Child #2 pushed Child #1 down the small indoor slide. LPA observed the play equipment and confirmed it is age-appropriate. Teacher #1 witnessed the incident but did not have sufficient time to intervene before the fall occurred. LPA observed Child #1\342\200\231s forehead and noted that the injury is improving. Parents were notified immediately, and an ice pack and wet towel were applied to Child #1\342\200\231s forehead. Child #1 was taken to the doctor, and no medical treatment was required. The center obtained a copy of the return-to-school note on 02/02/2026. Based on interviews conducted, this incident is deemed accidental. No deficiencies have been cited at this time. Facility is informed to report any unusual incidents within 24 hours to the Department. An exit interview was conducted, and a copy of this report was read and provided to Director Rosa Ayala Lopez, along with a copy of appeal rights and Notice of Site Visit.
