What the inspector wrote

On 09/14/23 at 9:30 AM Licensing Program Analysts (LPA) Alemoh met with Assistant Director , Krista Ribbons for an unannounced Case Management Self reported incident inspection that occurred at the facility on 09/05/2023. Upon entry to the facility, LPA Alemoh verified a census of 98 children in care with 23 staff present including teachers, and directors. LPA disclosed the purpose of the inspection and was granted entry by the Site Director. Description of the incident : On Tuesday, September 05, 2023 Staff reported that Assistant Director Issac was walking a group of students down the hallway to return them to their classrooms. When they turned the corner Child #1 remained in the hallway. Assistant Director conducted a name to face check and quickly realized that Child #1 was missing. Assistant director went to find Child #1. A parent had noticed that child #1 was left unattended and informed assistant teacher and was brought back into the classroom. Child was left unattended for under 2 minutes. The purpose of the inspection is to follow up with the facility regarding the incident dated 09/05/23. Based on LPA observation, interviews conducted with staff, documents gathered and record reviews, the preponderance of evidence standard has been met. Upon further investigation and video evidence, parents were observed being seen picking up and guiding their own children out of the hallway as well as observing child #1 in the hallway. Another parent entered the hallway to observe child #1 and informed the assistant teacher Emma that child #1 was in the hallway. Assistant teacher brought the child into her classroom where Assistant Director Isaac came to pick up the child from the classroom in less than two minutes. Day care child#1 was found in the hallway however legal parents/guardians picking up their child did observe the child in the hallway. Interviews conducted with staff indicated child was found by a parent and was notified and brought back into the classroom. Situation was handled by the facility by: Director Keelan interviewed all staff on site on the incident, signed off on the new policy and procedures such as Safety and Supervision Acknowledgement. There is one staff in the front of the line and one staff in the back of the line. Assistant Director Isaac was placed on administrative leave for a day. Staff training's are being conducted, last staff training was on August 18th. Per director staff training's are being conducted every month. No citation of deficiency will be issued today upon information reviewed. Exit interview conducted, a copy of this report, notice of site visit and appeal rights were provided to Assistant Director Krista Robbins, Assistant Director.
