On 12/07/2023, Licensing Program Analyst (LPA) Andrew Alemoh met with Director Keelan Pratt for the purpose of following up on an unusual incident report submitted to the Department on 12/01/2023. During the visit, LPA toured the facility and took a census of the children present. LPA observed 76 preschool age children with 15 staff with 3 administrators providing supervision. The incident report indicated that Child #1 who is enrolled in the preschool program was playing outside in the play yard area when another child (Child #2) pulled Child #1 pants down while playing outside climbing on the rock wall. During this investigation, LPA interviewed staff, children, requested relevant documents and was guided on a tour of the playground. Based on information provided and interviews conducted further investigation is needed. Exit interview was conducted, and a copy of this report was read and provided to the Director along with the appeal rights.
