What the inspector wrote

On 01/05/2024 Licensing Program Analyst Andrew Alemoh met with Director Keelan Pratt for the purpose of following up on an unusual incident report submitted to the Department on 12/01/2023. During the visit, LPA toured the facility and took a census of the children present. LPA observed 73 preschool children with 15 teachers and 3 administrators members providing supervision. The incident report indicated that Child #1 who is enrolled in the preschool program was playing outside in the play yard area when another child (Child #2) pulled Child #1 pants down while playing outside on the climbing rock wall. Facility records were reviewed, Staff, and Children were interviewed. Per interview with Staff and the day care children it was gathered that at the time of the incident the Staff member did not witness the act occur. Video evidence was obtained and observed that there were at least 4 teachers present on the outside play yard area. All 3 children at the time of the video evidence were observed to be playing and running around together on the play yard area. There is a blind spot in the video evidence that is unable to view the rock wall area however video evidence shows at least one or two teachers stationed and observing the rock wall area were the incident occurred. However teachers were unable to see the incident occur, children statements were obtained that the incident that occurred was an accident. A technical violation will be issued today See LIC9012 The facility plan of correction of the situation was: a notice letter was sent out to all staff to reassign staff members around the playground areas to cover the blind spots areas of the outside play yard area. The facility had a parents and teacher conference for all the children/parents who were involved of the incident. Staff training's on care and supervision were provided to staff. The facility called the parents the informed them of the situation. Director did speak with An exit interview was conducted, appeal rights discussed, and a copy of this report was provided to Director.
