What the inspector wrote

101152 (S)(1) (B)- School-Age Child" means any child who meets one of the following: (B) Is in a child care program providin g care and supervision exclusively to children enrolled in kindergarten and above. Please see attached LIC 809-D for citation. Exit interview was conducted with administrator Armand Wise. The administrator was provided a copy of the appeal rights (LIC 9058) and signature on this form acknowledges receipt of these forms. The Notice of Site Visit (LIC 9213) \342\200\223 must remain posted for 30 days during the hours of operation after each site visit made by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
