On September 12, 2024 at 11:00 AM, an unannounced Case Management visit was conducted by Licensing Program Manager I (LPM) Raul Navarro, and Licensing Program Analyst (LPA) Elicia Calvillo for the purpose of serving an Immediate Exclusion for Noresha Jayasundara, Dinushra Herath and Iresha Kumari Weerakkody Gedara , employees of this facility. LPM and LPA met with Gladys Villalobos, Director at the facility, located in Granada Hills, CA 91344. Licensee representative Gladys Villalobos, Director was served with the following by Licensing Program Manager, Raul Navarro: ORDER TO INDIV IDUAL OF IMMEDIATE EXCLUSION FROM ALL FACILITIES: Noresha Jayasundara, Dinushra Herath and Iresha Kumari Weerakkody Gedara , ORDER TO LICENSEE OF IMMEDIATE EXCLUSION FROM ALL LICENSED FACILITIES: Noresha Jayasundara, Dinushra Herath and Iresha Kumari Weerakkody Gedara, Appeal Rights Government Code 11522 LPM and LPA advised Facility Representative that Noresha Jayasundara, Dinushra Herath and Iresha Kumari Weerakkody Gedara , is excluded from Granada Hills Montessori Preschool 197750070, and any other licensed facilities by the Department of Social Services. Licensee and Noresha Jayasundara, Dinushra Herath and Iresha Kumari Weerakkody Gedara , have 15 days to submit an appeal. The address for the appeal was provided to Licensee. Licensing staff served the exclusion letter to Noresha Jayasundara, via Certified mail, Dinushra Herath in person, and Iresha Kumari Weerakkody Gedara was served by a Licensing Program representative from the Palmdale Regional Office, on 09/12/2024. PAGE 1 OF 2 Licensee representative Notified LPM and LPA that Noresha Jayasundara was ternintated on 07/03/2024, and Dinushra Herath and Iresha Kumari Weerakkody Gedara, were terminated on 07/05/2024 and have not been present at this facility since that date. Exit interview was conducted with Gladys Villalobos, Director. The Notice of Site Visit shall be posted for thirty (30) consecutive days.
