What Happened

Case management inspection following unusual incident report. On 2/12/25, staff member pulled child by ankle and spanked child on buttocks. Facility followed protocol by addressing issue with staff, notifying parent, and reporting to licensing. Type A deficiency issued for violation of child's personal rights.

Observations

  • Staff 1 pulled Child 1 by ankle and spanked Child 1 on buttocks on 2/12/25
  • Child 1 was having difficult day and not responding to directions/commands
  • Staff did hit Child 1 on hand
  • Facility followed protocol: addressed issue with staff involved, notified parent, notified licensing office
  • Child 1 was hit by staff member while in care, which violates personal rights
