What the inspector wrote

On February 20, 2025, Licensing Program Analyst (LPA), Calloway conducted an unannounced case management inspection to the above facility. LPA met with the Facility Director who granted access. LPA toured the facility with the Director and observed sixty-two-day care children and ten staff in care. On February 14, 2025, the Palmdale Region office received an Unusual Incident Report (UIR) stating on February 12, 2025, Staff 1 (S1) pulled Child 1 (C1) by the ankle and spanked Child 1 on their buttocks. LPA conducted confidential interviews that revealed C1 was having a difficult day on the day of the incident and not responding to directions or commands and S1 did hit C1 on the hand. Although the facility followed protocol and responded to the incident by addressing the issue with the Staff involved, notifying the parent, and the Licensing office, Child 1 was hit by a staff member while in care which is a violation of C1’s personal rights. There is one Type A deficiency cited for Personal Rights during this inspection. See 809D page attached to this report. Upon receipt of a Type A deficiency, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224. If these requirements are not met civil penalties per violation will be assessed. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Angelee Oster, Director at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
