What Happened
The incident follow-up found that a child was injured when the play-yard landscaping allowed a trip into a brick wall. Investigators linked the head injury to uneven terrain in the yard and issued a Type A citation.

- The play yard was uneven with dips in the hill leading to the brick wall
- Child #1 tripped while running and hit his head on the brick wall
