On Thursday, January 18, 2024 at 9:10 am., Licensing Program Analyst (LPA) Mayra Rivera arrived at the above facility for the purpose of conducting a Case Management Inspection for an Incident that was reported on 09/25/23. LPA met with Director Thalia Valdovinos who guided LPA Rivera on a tour of the facility. Teacher and child ratio were observed. LPA entered classroom 6 where the incident occurred. LPA observed the cubby and another cubby to be bolted to the wall. Per director, the cubbies were immediately bolted to the wall the same day of the incident. LPA observed the cubbies to be non-moveable or loose. LPA observed the child to be present at the facility. Per director the child received stiches and returned to the facility within two days. An exit interview was conducted with director Thalia Valdovinos and a copy of this report was provided to director along with appeal rights.
