On Thursday, November 13, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up in regards a self-reporting incident that occurred on October 27, 2025. LPA Rivera observed present staff fingerprinted cleared. LPA observed a total of 21 infant/toddlers and 6 staff present providing care and supervision. LPA observed the facility to be within ratio. On October 28, 2025, Palmdale Regional Office received an unusual incident report stating child walking to towards a table in the classroom, shuffled feet causing child to trip and hit head on the corner of the table. Child suffered a ½ inch cut around left eyebrow. Based on interviews conducted the incident does not appear to have been the result of any violation of Title 22 regulations or Health and Safety, therefore, no deficiencies were given. The incident was an accident. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with director Thalia Valdavinos. A copy of the report was also provided.
