What the inspector wrote

On Thursday, November 13, 2025, Licensing Program Analyst (LPA) Mayra Rivera conducted a Case Management inspection to follow up in regards a self-reporting incident that occurred on November 7, 2025. LPA Rivera observed present staff fingerprinted cleared, a total of 39 preschool children and 6 staff present providing care and supervision . LPA observed the facility to be within ratio. On November 7, 2025, Palmdale Regional Office received an unusual incident report stating child #1 was getting up from a bike and fell over hitting left ear on the seat of another bike. This caused a 1/2-inch cut from the outer tip of the ear inward. During this visit, LPA interviewed staff #1 who observed the incident, staff #2 who was present during the incident and child #1. LPA inspected the outdoor area. Based on interviews conducted the incident does not appear to have been the result of any violation of Title 22 regulations of Health and Safety, therefore, no deficiencies were given. The incident was an accident. A notice of site visit was given and must remain posted for 30 days. Failure to maintain posting as required will result in a $100.00 civil penalty. Exit interview conducted and report was reviewed with director Thalia Valdavinos. A copy of the report was also provided.
