What the inspector wrote

On April 30, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility and met with Michelle Rodriguez, acting director as the representative who granted access. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on April 20, 2026. LPA toured the facility with the representative and observed three small infants and fourteen toddler day care children and five staff in active care. On April 17, 2026, at 10:47 AM, the reported information involved a Child 1 (C1) transitioning from the outside playground to the inside when the child tripped over a staff’s foot, landed on the pavement face first, and suffered a bloody nose. The parent was notified and C1 began going in and out of consciousness then 911 was notified. The paramedics arrived and took C1 and their parent via airlift to the children’s hospital. Based on interviews and record review, the staff notified the paramedics, C1’s parents, and Community Care Licensing (CCL) in a timely manner. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, was given to Michelle Rodriguez, representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
