What the inspector wrote

On June 25, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on June 11, 2025. LPA toured the facility with the representative and observed one hundred six children and sixteen staff in active care. On June 10, 2025, the reported information involved Staff leaving Child 1 (C1) unattended, alone in the hallway transitioning to another classroom and did not account for C1. LPA conducted interviews with the relevant parties involved. LPA reviewed staff and children files and completed a safety inspection of the hallway. There is a door in the hallway leading outside that was observed closed with a safety alarm. Per representative, the video surveillance is only live feed. Based on interviews it was determined that Staff was doing their name to face checks, Staff engaged another child that was having a hard time transitioning, Staff took that child into the classroom, Staff closed the door to the classroom, and left C1 alone in the hallway. A parent dropping off their child found C1 in the hallway and took C1 to their classroom. The facility representative notified C1\342\200\231s parents, and Community Care Licensing (CCL) and conducted interviews with the staff. Based on observation, the school was secure and locked during LPA walk through, and the child was not able to leave the premises. Although the staff took appropriate action, the care and supervision violation did occur. There is one Type A deficiency cited during this inspection. See the LIC 809D page attached to this report. Upon receipt of a Type A deficiency, a copy of the licensing report must also be posted for 30 days. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee must obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file. Copies of the reports must be provided to each parent when a Type A violation is cited along with Acknowledgment of Receipt of Licensing Reports LIC 9224, and remain in the file for three years. If these requirements are not met civil penalties per violation will be assessed Exit interview was conducted and a copy of this report was read, a Notice of Site Visit, and Appeal Rights were provided to Shirley King, Facility Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
