What the inspector wrote

On December 17, 2025, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted to the Palmdale Regional Office on December 11, 2025. LPA arrived and the facility director was not available during the inspection. LPA toured the facility with the assistant director and observed seventy-three-napping day care children and twelve staff. All adults 18 years and over have Criminal Record Clearance (DOJ) and Child Abuse Central Index Clearance and are associated with the facility. On 12/11/25, the reported information involved an incident that occurred on the playground with two children Child 1 (C1) and Child 2 (C2) where C1 fell, hit the teeter totter, and sustained an injury that split their lip open. C1 went to the doctor and required stitches to their lip. LPA conducted interviews with all relevant parties involved. LPA reviewed records and conducted a safety inspection of the playground. At this time further follow up is needed. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit were provided to Krista Ribbins, at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
