What the inspector wrote

On January 15, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility. LPA met with the Facility Director who granted access. LPA and Director toured the facility and there were seventy-one preschool children in active care and six staff. The purpose of the inspection was to follow up on self-reported UIR. On December 11, 2025, the reported information involved an incident that occurred on the playground with two children Child 1 (C1) and Child 2 (C2) where C1 fell, hit the teeter totter, and sustained an injury that split open their lip and caused C1 to need stitches to their lip. During today\342\200\231s inspection, LPA conducted interviews with relevant parties involved. As a result, the incident appeared to be an accident that happened quickly while children were playing outside and with staff observation of the area. As a result, the Staff stated they no longer allow the children to wait near the teeter totter or play on the teeter totter and they will provide supervision to ensure the children follow the new changes. There were no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, and a Notice of Site Visit was given to Shirley King, at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting will result in a $100 civil penalty.
