What the inspector wrote

On April 30, 2026, Licensing Program Analyst (LPA) Calloway, made an unannounced case management inspection to the above facility and met with Michelle Rodriguez, acting director as the representative who granted access. The purpose of the case management was to follow up on a self-reported Unusual Incident Report (UIR) submitted on to the Palmdale Regional Office on April 20, 2026. LPA toured the facility with the representative and observed eighty-five preschool children and ten staff in active care. On April 16, 2026, the reported information stated during pick up and clean up time around 5:50 PM., a Child 1 (C1) became unresponsive with the staff while inside the classroom and 911 was notified and the parent was notified. The paramedics arrived and took C1 with their parent to the hospital. On April 17, 2026, at 10:00 AM., C1’s parent arrived at the day care and informed the staff C1 was found to have a fever. Based on interviews and record review, it was determined that C1 suffered a medical emergency and the staff responded and followed protocol by contacting the paramedics for further emergency medical treatment, the parents, and Community Care Licensing (CCL) in a timely manner. There are no deficiencies cited during this inspection. Exit interview was conducted and a copy of this report was read, a Notice of Site Visit (LIC 9213), was given to Michelle Rodriguez, Representative at the facility. A Notice of Site Visit must remain posted for thirty (30) consecutive days. Failure to maintain posting may result in a $100 civil penalty.
