What the inspector wrote

On Wednesday, January 28, 2026, Licensing Program Analyst (LPA) Hanna Cha met with Director Lilit Khachatryan to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the Director and toured the facility. At time of arrival, LPA observed nine staff members providing care and supervision to 51 preschool-aged children. The current days and hours of operation are Mondays to Fridays; 8:00am-5:00pm. Physical Plant/Indoor/Children\342\200\231s Area: The facility operates out of five classrooms, separated by age groups. There are two classrooms for ages 2-3, two classrooms for ages 3-4, and one classroom for ages 4-5. The office is located to the right from the front gate which remains locked. The preschool classrooms were observed for age-appropriate toys/equipment, books, and supplies for children. LPA observed individual cubbies labeled with children\342\200\231s names. The children provided with blankets and cots for napping. The diaper changing tables were observed to be in clean and sanitary condition. The cleaning compounds made inaccessible to children. Per Director, smoking is prohibited on the premises. Per Director, there are no firearms/weapons stored on premises. The carbon monoxide alarms and smoke alarms were observed. The isolation area for children is provided in the office. Bathroom: LPA observed three children restrooms for safe and sanitary operating conditions with no hazardous materials. Toilet paper, paper towels, and soap available to children. The toilets and sinks in operable condition. Kitchen: LPA observed the kitchen for safe and sanitary conditions. The kitchen made inaccessible by a door that remains closed. The kitchen includes a refrigerator/freezer, sink, food preparation counters, and pantries. Per Director, meals are cooked and prepared in the kitchen. LPA observed menus updated as needed and an allergy list in kitchen. Outdoor/Children\342\200\231s Play Area: LPA observed three outdoor play areas for safety, cleanliness, good repair, age-appropriate toys/equipment, cushioned floors, and shady areas. All outdoor play areas enclosed by fencing and gates. Per Director, gates remain closed. Per LPA observation, there are no pools or spas on the premises. File Review: LPA reviewed six staff files and verified fingerprint clearance of all staff present. Per record review, two out of six staff files did not include proof of immunizations (MMR, Tdap). LPA informed Director that all employed staff files must include proof of immunizations against pertussis, measles, and influenza. Director will provide proof of immunizations for all four staff files to the department by POC due date. A Type B citation issued. Per record review, six staff files did not include proof of a Health Screening (LIC 503) verifying good physical health performed or under the supervision of a physician. All six staff files did include proof of TB testing. Director will ensure that all employed staff will complete a health screening and provide proof to the department by the POC due date. A Type B citation issued. LPA reviewed five children files which contained all required documentation. The disaster drills last completed on 01/2026. Per record review, the lead testing for water last conducted on 10/03/2023. To improve the quality and value of the new inspection process, a survey may be sent to the email address provided. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP. LPA referred licensee facility representat.
