What the inspector wrote

On 11/10/2025, LPA Hanna Cha amended report made on 08/28/2025 to change final determination of follow-up inspection. On Thursday, August 28, 2025, Licensing Program Analyst (LPA) Hanna Cha met with Director Melinda Sims. The purpose of the inspection was to conduct an unannounced case management follow-up inspection for an Unusual Incident Report (UIR) received at Palmdale Regional office on 08/04/2025 regarding facility gate left open and a stranger walking into the facility office. LPA Cha disclosed the purpose of the inspection to the Director. Upon arrival, there were four preschool children in care with two teachers providing care and supervision. The current days and hours of operation are Mondays through Fridays; 9:00am-3:00pm. During the visit, LPA Cha interviewed children, parents via telephone, and interviewed staff. LPA Cha completed a safety inspection of the facility at approximately 11:15am. No safety concerns observed. The incident was determined to be an isolated incident. No follow-up is needed. A Notice of Site Visit was given and must remain posted for 30 days. Exit interview was conducted and the report was reviewed with Director Melinda Sims.
