On 10/13/2023 at 10:40am, Licensing Program Analyst (LPA) Andrea Pittman and Licensing Program Manager (LPM) Mariela Ramon conducted an unannounced case management visit at the facility and was met by the Director Vonda Perry who permitted entry to the facility. LPM toured the facility with the Director according to the facility sketch. Upon arrival, LPA observed 7 children with 2 staff members providing care and supervision. On 8/3/2023, the Director reported an Unusual Incident to the office via phone stating that she had a parent report that their child used profane language which the child informed the parent that they allegedly learned from Staff 1. After observations, interviews, and record reviews, the following information was revealed about the incident which led to the submission of the unusual incident report: During this investigation, LPA received pertinent documents related to this investigation, which included the facility’s staff roster, children’s rosters, and other relevant documents. It was revealed that there have been no reportable incidents to the center’s director and staff of the staff using profane language in the presence of the children. The unusual incident was followed up and it was determined that there was no violation of Title 22 Regulations that occurred in this incident. The facility is encouraged to continue to report unusual incidents. An exit interview was conducted, and a copy of this report was read and provided to Director Vonda Perry along with the Notice of Site Visit and Appeal Rights.
