What the inspector wrote

An Informal Office Meeting was conducted on June 3, 2025, at 10:00 AM at the Palmdale Regional Office (PRO). Present during the meeting were Regional Manager, George Mingle, Licensing Program Manager (LPM) Mariela Ramon, Licensing Program Analysts (LPAs) Annelise Villa and Sherell Braddock, and Directors Vonda Perry and Africa Jones. Licensee Rivka Jones was present during the meeting via telephone. The goal of this meeting is to address recent concerns with the Directors of the childcare center and to assist the Licensee to achieve and maintain facility compliance with Title 22 California Code of Regulations as it aligns with the mission of at the Department. During this Informal meeting, the Department addressed concerns with the operation of the facility. During a recent interaction with the facility, facility staff were not fully cooperative with the Department in sharing requested information to assist with the complaint investigation. Facility staff failed to provide names of witnesses and video surveillance videos as requested. As a result, this office meeting is being held to discuss concerns and Title 22 regulations related to these issues. This office meeting is being conducted to present a training opportunity to Licensee and Directors. Failure to provide access to surveillance footage, which is considered relevant evidence to the complaint, obstructs the licensing agency’s responsibility to determine if any violations occurred and compromises regulatory oversight. Continued on LIC 809-C The licensing agency will continue to seek compliance and recommends that the director be reminded of their obligation under Title 22 Regulations to fully cooperate with all aspects of licensing evaluations and investigations. Continued non-compliance may escalate the matter to legal or administrative proceedings. The applicable regulations were provided during this meeting: Admission Procedures and Parental and Authorized Representative’s Rights Accountability Limitations on Capacity Reporting Requirements Admission Agreements In response to these discussions, the licensee has agreed to the following: Will cooperate with licensing investigations. Will provide a safe and healthful environment. The facility will ensure to report unusual incidents - Reporting Requirements Will always comply with all applicable Title 22 regulations, including Title 22, Section 101200. Inspection Authority of the Department. Parent Handbook will need to be modified to address termination procedures During this office meeting, the following training videos were played for Directors: Community Care Licensing Inspection Authority Your Rights as a Child Care Licensee The above training, as well as other child care provider videos, can be accessed at: https://ccld.childcarevideos.org The licensee confirmed understanding of these requirements and had no further questions. An exit interview was conducted, and a copy of this report was read, printed, and emailed to the licensee.
