On 07/30/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Lead Teacher Jardi Solares, who granted access to the facility. The purpose of the inspection was to conduct an unannounced case management inspection for a UIR received at Palmdale RO on 06/10/2025. LPA disclosed the purpose of the inspection. Upon arrival, there were a total of 19 infants in care, including 8 staff members present. Description of incident: On 06/09/25, around 9-10AM, Teacher Assistant #1 was holding infant#1 and bumped into furniture and fell with infant #1 in their arms. LPA Tamayo interviewed staff involved, however Teacher Assistant #1 was not present at the time of inspection. A follow up case management-incident inspection will be conducted at a later date to obtain additional information. An exit interview was conducted, and a copy of this report was read and provided to Lead Teacher, along with a copy of the Notice of Site Visit.
