What the inspector wrote

On 08/12/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Lead Teacher Jardi Solares, who granted access to the facility. The purpose of the inspection was to conduct a follow up unannounced case management inspection for a UIR received at Palmdale RO on 06/10/2025. LPA disclosed the purpose of the inspection. Upon arrival, there were a total of 14 infants in care, including 8 staff members present. Description of incident: On 06/09/25, around 9-10AM, Teacher Assistant #1 was holding infant#1 and bumped into furniture and fell with infant #1 in their arms. During interviews with staff and Parent #1, it was confirmed that the center notified the parent of the incident after it occurred. Assistant Teacher #1 was holding child #1 when they tripped, but the teacher was able to shield the child during the fall. According to parent #1, the child was not injured, and the parent expressed no concerns about the facility or its staff. In response to the incident, staff have relocated the outdoor furniture to prevent a similar situation in the future. Based on the information gathered, the incident was determined to be accidental, and no deficiencies will be cited. An exit interview was conducted, and a copy of this report was given to Lead Teacher Jardi Solares, along with a copy of her appeal rights and Notice of Site Visit.

    Personal Rights 101223(a)(1): The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons.This requirement was not met as evidence by: Assistant Teacher #1 did not observe Teacher #1 slap the hand of child #1, however Teacher #1 did pull a book out of child #1 hand in an aggressive manner, which poses a potential health and safety risk to children in care.
