What the inspector wrote

Based on the information obtained through interviews, record reviews, and observations, it could not be determined whether the above allegations occurred as reported. Based on information obtained, and LPA observations the above allegations are deemed Unsubstantiated. An Unsubstantiated finding means, the above allegations may have happened or are valid, but there is not a preponderance of evidence to prove or disprove that the alleged violations occurred. This inspection was conducted in person. LPA read the report with Assistant Director, Nicole Medina and provided a copy of the report, Appeal Rights, and a Notice of Site Visit was given and must be posted for 30 days. Exit interview conducted.
