What the inspector wrote

On Thursday, May 15, 2025, Licensing Program Analyst (LPA) Braddock conducted an unannounced Case Management Inspection at the facility. LPA disclosed the purpose of the inspection to the Director Chauncy Smith (S1). LPA and S1 completed a safety tour of the facility. When LPA arrived at the facility, LPA observed 6 infants, 17 preschool and 0 school age children in care with 6 teachers , assistant director and director. Based on LPA interviews, and documents obtained there was an allegation that occurred in early January 2025, alleging: Facility staff left a day care child (C1) inside of a parked vehicle. Director (S1) failed to inform the department of the allegation after speaking with C1 guardian. S1 states “I kind of let it go, because I didn’t have any proof” referring to the reason he never addressed the allegation. A Type B deficiency will be cited for failing to report the incident under Title 22 section code- 101212(d)(1)(C) Reporting Requirements Additionally, records review conducted revealed that the center is not maintaining a current roster of children receiving care in facility. Review shows C1 name was not listed on any of the rosters provided to LPA Braddock, however Staff 1 and 2 stated C1 attended the day care center. The following Type B deficiency will be cited under Health and Safety Code1596.841. Deficiencies cited: (See LIC 809D). The following Type B deficiencies are being cited in accordance with Title 22 of the California Code of Regulations and/or Health & Safety codes. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. This requirement was not met as evidenced by: Based on interviews conducted, staff failed to ensure that all children were removed from the van after school pick up. C1 was left alone outside the facility sleep in the van which poses an immediate health, safety or personal rights risk to persons in care.

This requirement was not met as evidenced by: Based on interviews and record reviews the licensee did not ensure the above allegation that C1 was Left inside of a parked vehicle was reported to CCL, which poses a potential risk to the health safety and personal rights of the clients in care.
