101212(b)- Reporting Requirements (b) The name of the child care center director, and any fully qualified teacher(s) designated to act in the child care center director's absence, shall be reported to the Department within 10 days of a change of child care center director or designee(s). Based on interviews and record reviews the original, designated director never worked at the facility and the Licensee did not notify Licensing of the changes with the Director which is a potential health, safety, or personal rights risk to the persons in care.

This requirement was not met as evidenced by: Based on interviews, in the evening, the infant day care children are co-mingled with the preschool and school aged children. The children are strapped inside a multi-seated stroller in the lobby, or put in one classroom, or outside on the same play yard which is an immediate health, safety, or personal rights risks to the persons in care.
