What the inspector wrote

On 01/10/2025 at 11:35AM Licensing Program Analysts (LPAs) Justeene Tamayo and Hanna Cha met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conduct an unannounced follow up case management inspection for a UIR received at Palmdale RO on 12/17/24. The center went on Winter Break until the beginning of January, therefore LPA was unable to investigate the UIR at the time of occurence. LPA disclosed the purpose of the inspection. Upon arrival, there are 32 preschool children in care, along with 9 teachers. Description of Incident: On 12/16/24, Child #1 fell on the playground and scratched the left side of their face. Doctor released child #1 to return to daycare on 12/17/24. During this inspection, LPAs interviewed child #1 and obtained additional information due to child #1 was not present at the initial case management UIR inspection on 01/10/25. During interviews with children #1-5, Teachers #1-3 and the Director, it was revealed Teacher #1 was providing care and supervision to child #1. Child #1 was running/playing on the playground and accidentally tripped over their feet, which resulted in child #1 sustaining a scratch on the left side of their face. Teacher #1 did not have enough time to intervene, however as soon as child #1 fell, a cold compress was applied to child #1 face, and the parent of child #1 was immediately notified. Parent picked up child #1 within 20 minutes. LPAs observed the playground area to be free of hazardous/tripping hazards, therefore this incident is deemed accidental. No deficiencies were cited at this time. The director was informed to continue to report any unusual incidents to the Palmdale Regional Office within 24 hours. An exit interview was conducted, a copy of this report was provided to Director, along with her appeal rights and Notice of Site Visit.
