What the inspector wrote

On 08/20/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conduct an unannounced case management inspection follow up for a UIR received at Palmdale RO on 04/08/25 and occurred on 04/07/25. LPA was unable to conduct a visit at the time due to gastrointestinal outbreak at center. Upon arrival, there were 38 preschool age children present, along with 2 Lead Teachers, and 6 Teacher Assistants. Summary of Incident: Preschool Classroom 5 was playing outside. There were 2 teachers and 14 children present at the time of occurrence. Child #1 went through 2 gates and wondered outside of the facility. Child #1 was crying. Teacher #1 heard him crying and saw child #1 outside of the gate. Teacher #1 then went to get child #1. During interviews, it was confirmed that Teacher #1 and Teacher #2 took the children outside for outdoor activities. Child #1 became upset because they wanted to go home with their parent. While Teachers #1 and #2 were attending to other children, Child #1 left through the first gate without supervision for about 2-5 minutes. Teacher #2 then heard Child #1 crying as the child was attempting to open the second gate and quickly went to retrieve child #1. Based on the information gathered, the teachers could not explain how Child #1 managed to exit through the first gate. As a result, the facility has been issued a Type B Citation: Responsibility for Providing Care and Supervision 101229(a)(1). Please see LIC809-D for deficiency page. An exit interview was conducted, and a copy of this report was read and provided to Director Rosa Ayala Lopez, along with a copy of her appeal rights and Notice of Site Visit.
