Inspector’s narrative

What the inspector wrote

On 10/03/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conduct a follow up unannounced case management inspection for a UIR received at Palmdale RO on 09/12/25 and occurred on 09/12/25. Upon arrival, there were 39 preschool age children present and 9 preschool teachers. Summary of Incident : Parent #1 of a child #1 stated she witnessed teacher #1 pull and squeeze child #1 hand. However, Parent #1 then stated that child #1 stated teacher #1 pulled and squeezed child #1 hand. Based on interviews with Teacher #1, Assistant Teacher #2, the Director, Child #1, Children #2\342\200\2234, and other relevant parties, it was revealed Teacher #1 stated they do not recall this incident in question. Assistant Teacher #2 also reported they did not witness the incident. During an interview, Child #1 stated that Teacher #1 squeezed their arm, but clarified that it did not hurt and that Teacher #1 did not do anything to them. Children #2\342\200\2234 reported they did not observe any unusual interaction between Child #1 and Teacher #1. Additionally, Children #2\342\200\2234 expressed that they have no issues with Teacher #1 and that they enjoy attending the daycare. Please see LIC809-C for continuation page. Based on the information obtained, there were no violations of Title 22 regulations. Facility is informed to continue to report any unusual incidents to the Palmdale Regional Office within a timely manner. No deficiencies have been cited at this time. An exit interview was conducted, and a copy of this report was read and provided to Director, along with a copy of her appeal rights and Notice of Site Visit.
