What the inspector wrote

On 10/24/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Rosa Ayala Lopez, who granted access to the facility. The purpose of the inspection was to conduct a follow up unannounced case management inspection for a UIR received at Palmdale RO on 10/17/25 and occurred on 10/16/25. Upon arrival, there were 40 preschool age children present and 8 preschool teachers, along with Director. Summary of Incident : Child #1 was playing on a trampoline with a bar across when child #1 hit their head on their left eyebrow on the bar of the trampoline. Child #1 sustained a bruise and cut on the left side of their head. Based on interviews with teacher #1, assistant teacher #1, child #1, Director, and other relevant parties, it was revealed this incident occurred accidentally and quickly. LPA observed 2 mini individual trampolines with a safety bumper (noodle) on each of the bars to ensure safety. Teacher #1 had visual observation of child, however, teacher #1 was attending to another child during the time of occurrence. Child #1 got off of the trampoline and sat on the green bench. Child #1 then began to cry. Once teacher #1 heard child #1 crying, teacher #1 immediately attended to child #1 and took child #1 inside. An ice pack was applied to child #1 injury, and parent corroborated that they were notified immediately after this incident. Child #1 has resumed to normal activities. No doctors visit was needed. Please see LIC809-C for continuation page. Based on the information obtained, this incident is deemed accidental and occurred quickly. No deficiencies will be cited at this time. An exit interview was conducted, and a copy of this report was read and provided to Director on this date, along with a copy of her appeal rights and Notice of Site Visit.
