On 12/10/2025, Licensing Program Analyst (LPA) Justeene Tamayo conducted an unannounced Case Management visit. The purpose of the visit was to inspect the finished completion of the outdoor shaded structure that was professionally installed for the preschool outdoor activity space. LPA met with Director Rosa Ayala Lopez. Upon arrival, there were 43 preschool age children and 9 teachers present. A temporary waiver was approved on 11/12/25 , allowing preschool-age children to continue using the outdoor play yard while the new shade structure was installed. During the installation period, children used the left side of the play yard at alternating times. During today\342\200\231s visit, LPA Tamayo observed the completed blue shaded structure, which was professionally installed by Safe Play and finished on 12/08/25 . The structure was found to be in stable condition. There are 6 pillars with safety cushions to prevent any bumping accidents. Toys and outdoor play equipment for preschool-age children were present, and no hazardous items or unsafe equipment were observed. LPA Tamayo took photos of the completed shade structure to update the facility file. The center will now return to its normal outdoor activity schedule, and the temporary waiver is no longer needed. An exit interview was conducted, and a copy of this report was read and provided to Director Rosa Ayala Lopez, along with a copy of her appeal rights and Notice of Site Visit.
