What the inspector wrote

On 3/04/2025 Licensing Program Analyst (LPAs) Isabel Ortega and Mayra Rivera conducted an unannounced complaint inspection to on the above allegations. LPAs met with Assistant Director and Darrell Maston Licensee, toured the facility. Upon arrival LPA observed 31 children and 6 Staff providing care and supervision. During the course of the investigation, LPAs conducted interviews, and completed a walk through observation. LPAs completed visual observations and reviewed documents relevant to the complaint allegations listed above. These allegations are deemed SUBSTANTIATED, and a citation was issued (See LIC 9099-D for cited deficiency). A finding being substantiated means that the allegation was valid because the preponderance of the evidence standard has been met. This facility was cited a Type B for no qualified Assistant Director in accordance with Title 22 of the California Code of Regulations and Health & Safety codes. Facility was given a technical violations for no diaper changing table/changing mat, sink, or water available for hand washng after diaper changes in the Yellow room, a hole is present on the wall by the hallway and drywall by the yellow door is chipped, the yellow classroom, designated for children under two years old, is being utilized for storage, according to facility and record reviews currently there are no children enrolled under 2 years of age. Exit interview was conducted, a copy of this report, appeal rights and a notice of site visit were provided to facility.

LPAs reviewed staff files and Staff #10 is not fingerprint cleared. This poses an immediate health and safety risk to children in care.

  • TEACHER QUALIFICATIONS AND DUTIES

    101216.1 Teacher Qualifications(c) To be a fully qualified teacher, a teacher shall...: (1) Twelve...units in early childhood education or child development..., with passing grades, at an accredited... college or university. This requirement was not met as evidence by: Based on interview, observation, and record review, facility staff are not able to provide required CDE units, record keeping orientation and preventative Health and safety certificate to be a qualified Director. Facility admitted be acting as the Assistant Director which poses is a potential Health and Safety Risk to children in care.

  • ARTICLE 1. General Provisions and Definitions

    Based on record review the licensee did not comply with the section cited above in having 7 staff members not submit prood of immunizations which poses a potential health, safety or personal rights risk to persons in care.

    Based on record review, the licensee did not comply with the section cited above in 3 staff not having proof of Child Abuse Mandated Certificate which poses a potential health, safety or personal rights risk to persons in care.
